Terminal 1 gives you access to the Sala Cibeles. Depending on the time of day, it might get crowded as it shares with Korean airlines, and a couple of other rather large flights, but at other times it's positively empty. Catering is so-so, better than you'd find in a bmi lounge, but not as good as, say, the MLL in YYZ. A bit of a hike to the far end of T1, but much nicer than, say, the contract lounges in MXP.
All of the terminals have Star Alliance lounges, I was just there last week and none were very busy and were all quite nice. As imverge mentioned look for the Spanair lounges, although some were not clearly marked and looked the same as one of the other airline alliance lounges. You'll know you're at the right one as the list of airlines will be listed once you get closer to the lounge entrance.
The lounge in Terminal 1 was particularly loud as the front desk attendant was calling every flight 3 times. (pre-board, board, last call).
If I recall correctly, the wine was complimentary. Snacks are limited, so it helps if you like olives. :)
