You copy the stylesheet into your About Me section and save it. Unless its like a div overlay - then the website that hosts the layout should TELL you where to place

it is called a default layout whenever there is a long rectangle in the middle..so just search default layouts

it depends on where you get the layout. try looking on google for some myspace layouts or type in small layouts.. i dont know what to say about the lettering though.. i have the same type of layout with the long black box. i just found it on a layout provider page.
