  • Mine's actually an improvement - I got a wallet sized one. Better than my original that was an 8.5 x 11 sheet of paper that looked like someone had just typed it up on a typewriter.

    ChrisA330,

Good reference to the DEC's Web site and the English-language forms. Because alot of our staff move around for work we're always applying for passports (and for birth certificates as a first step); we must use the downloaded forms and fax-in request system about 2/3 times a month; works very well 95% of the time.

One warning however, the Passport Office WILL NOT under any circumstances accept the plasticized wallet-size version (no matter when issued, i.e. pre-1994 or post-1994) of a Quebec birth certificate in support of a passport application; you need the full size certificate.

  • I had the same question about passport support and so decided upon the larger version. The application form states that the smaller version cannot be used to support a minor's passport application, but doesn't state that the smaller one is OK for an adult. Not wanting to risk it, I chose the larger one. It's frustrating!

  • Ditto, it took about three weeks.

It sucks that it no longer fits in your wallet! I got the full version (81/2" X 11" including tear off portion).

  • I've just gone through this process and it was relatively painless.

The application form can be found here: http://www.etatcivil.gouv.qc.ca/publications/DemandeAnglais.pdf
(You need to request a certificate or abridged certificate)

More information on the application process found here:

My new cert isn't plasticized, it's printed on special paper with raised ink - similar to currency. I'm going to have a problem with that? Haven't taken my passport application in yet,

The passport office will ask for the larger version as described by NoahVail ( the one with the tear-off portion that indicates the request number)

I believe that you could have requested both versions for add $$ (of course) : the wallet one that you can carry and the larger version that is needed only for special govt apps. That's what my husband did back in 1998 and the larger version come out from the filing cabinet when he needs to renew his passport.

  • How much does getting a small tattoo on your hip/stomach hurt?
Do anyone else have an itchy anus? ?
